You will prepare the Power of Attorney based on a number of questions and answers. As you work through the documents have in mind the information provided in this guidance note.
On the next page you will start to complete the document. Please note that it is possible to prepare the document without having to pay for it first by clicking the preview box. When you are happy with the document you can pay for it. Your Power of Attorney is then e-mailed to this office. We will then telephone you to discuss your document in more detail. If any changes need to be made we will discuss any additional costs involved before finalising the document.
You can appoint someone to make welfare decisions for you in the event of mental incapacity. Decisions could be about:
- life sustaining treatment
- where you live and who you live with
- complaints over medical treatment
You can also select replacement attorneys if your primary attorney is unable to act. In the case of couples this could be the children.
If you have previously prepared an Enduring Power of Attorney this does not give your attorney any powers over welfare decisions. You may wish to consider preparing a Welfare Lasting Power of Attorney in addition to this document.
