You will prepare the Living Will based on a number of questions and answers. As you work through the document have in mind the information provided in this guidance note.

On the next page you will start to complete the document. Please note that it is possible to prepare the document without having to pay for it first by clicking the preview box. When you are happy with the document you can pay for it. The Living Will is then e-mailed to this office. We will then telephone you to discuss the document in more detail. If any changes need to be made we will discuss any any additional costs involved before finalising the document.

You will need to consider the following:

• Whether you wish to refuse life sustaining treatment
• whether you have any specific request concerning particular illnesses
• whether a specific person to be consulted about your treatment
• whether a specific person to be contacted in the event of your death

Although some of these issues could be covered in a Welfare Lasting Power of Attorney a Living Will covers particular requests for specific treatments.

It is important that you provide your General Practitioner with a copy of this document and review it regularly.
